The Application Instruction Guide contains instructions for completing the Florida Forever Grant Application Form (see below). Please review all instructions in the manual before preparing the application. An Application Completeness Checklist (Word) should be completed before submitting the grant application. The purpose of the checklist is to minimize mistakes or omissions in preparing the application. Applicants may also fill out a project self-score check sheet (Word) prior to submitting the application. This is a helpful but optional tool to determine if a project will receive a competitive evaluation score. If you have questions about the Florida Forever Program, review of grant applications, project evaluation reports, and the grant selection process, please contact the evaluation staff member listed on the program contacts page.

- Low Income Neighborhood: Data from the U.S. Government, used by the U.S. Department of Housing and Urban Development (HUD), to determine the income level of census tracts in Florida. To find information for your county, choose "Florida" from the list of states and click on "Retrieve by County." Then choose your county and click on "Get Census Demographic." (Criteria 3(c)4)
- Rural Areas of Critical Economic Concern: List of counties and communities designated as Rural Areas of Critical Economic Concern. (Criteria 3(c)5)
